Independence.
We help older adults keep their
independence as long as possible. We
help adults recover following
hospitalization or nursing home care. We
assist younger adults with care needs
related to chronic, long-term
conditions. And, we offer services to
help the families of all those under our
care.
Services Include:
Skilled nursing services
Provides treatments ordered by the
physician, instruction in special diets,
new medications and activities of daily
living.
Skilled rehabilitation including
physical therapy, occupational therapy,
& speech therapy
Provides therapeutic exercises to assist
persons to reach their highest level of
functioning.
Medical social services and care
management
Assists individuals to understand
consequences of illness and disease,
assists patients and families to cope
with changes in life situations and
makes referrals to other community
services.
Home health aide/personal care
services
Assists with personal care, medication
reminders, exercises and light
housekeeping.
Home delivered meals
Delivers attractive, nutritious meals to
the homes providing one-third of the
daily food requirement.
Homemaking
Provides homemaker services including
cleaning, laundry, meal preparation and
shopping.
Transportation
Provides round trips for medical
appointments, the airport and personal
appointments, and group trips to
sporting events, shopping trips,
cultural activities, restaurants and
church services.
Respite care services
Provides an opportunity for caregivers
who have major responsibility for
patient care to have time for
themselves.
Companion care
Provides stimulating activities to
clients consistent with the patient's
physical and mental orientation.
